Optimal Diabetes Care-
According to the Centers for Disease Control and Prevention (CDC), over 30 million adults aged 18 and older (12.2 percent of all U.S. adults) had diabetes in 2015. The percentage of adults with diabetes increases with age, reaching a high of 25 percent among people over 65 years of age. According to the American Diabetes Association, diabetes care that includes an emphasis on blood pressure, lipids, glucose, aspirin use and non-use of tobacco will maximize health outcomes.
This measure reflects the percentage of patients with diabetes (Type 1 and Type 2) ages 18-75 who reached all five of the following treatment goals to reduce cardiovascular risk:
• Blood pressure less than 140/90 mmHg
• Hemoglobin A1c (HbA1c) less than 8
• On a statin medication, unless allowed contraindications or exceptions are present
• Daily aspirin use for diabetes patients with a co-morbidity ischemic vascular disease unless contraindicated
• Documented tobacco-free status
The Minnesota statewide rate for Optimal Diabetes Care is 45 percent.
Optimal Vascular Care
Cardiovascular disease (CVD) is the leading cause of death for men and women in the United States. Considering the overall toll of cardiovascular disease, measures that assess clinical care performance are vital to reducing the negative impacts of CVD.
This measure reflects the percentage of patients ages 18-75 with vascular disease who reached these four treatment goals to reduce the risk of cardiovascular disease:
• Blood pressure less than 140/90 mmHg
• On a statin medication, unless allowed contraindications or exceptions are present
• Daily aspirin use
• Documented tobacco-free status
The Minnesota statewide rate for Optimal Vascular Care is 62 percent.
