– Have you traveled internationally within the last 14 days?
– Have you had close contact with or cared for someone diagnosed with COVID-19 with the last 14 days?
– Have you experienced any cold or flu-like symptoms in the last 14 days?
If you answered yes to any of the questions, do not request an appointment and instead call the Member Relations Team at 1-800-850-7676.
