ALTRU HEALTH SYSTEM AND GRAND FORKS PUBLIC HEALTH PARTNER FOR 2022 COMMUNITY HEALTH ASSESSMENT

Altru Health System and Grand Forks Public Health are excited to announce the community survey to inform the 2022 Community Health Assessment is now open. Completed every three years, the Community Health Assessment is a comprehensive process to identify the areas that impact our communities’ health and well-being that are in greatest need of improvement. The assessment is facilitated by Altru and Grand Forks Public Health and guided by an Advisory Committee including numerous community stakeholders. The community survey provides the Advisory Committee with the opportunity to hear from residents and is an important part of this process.

The community survey assesses the health of residents, identifies health service deficiencies and proficiencies, and provides insights about residents’ opinions, attitudes, and beliefs on health issues that affect them and their community.

Everyone 18 years and older living in Grand Forks County in North Dakota and Polk County in Minnesota is invited to complete the survey to help the Advisory Committee gain a better understanding of the area’s current health disparities. The survey will be available until Monday, May 9, and is being conducted in partnership with the University of North Dakota Master of Public Health program.

The survey is confidential and can be accessed on Altru Health System’s website (https://www.altru.org/about-us/community-health/) and Grand Forks Public Health’s website, or a paper copy can be obtained at Grand Forks Public Health, 151 S. 4th St. Ste. N301 or the information desk at Altru Hospital, 1200 S. Columbia Rd. Paper copies can also be obtained at many locations in Grand Forks and Polk Counties, including at schools, senior centers, and health clinics.
