Choice Therapy in Crookston will be honoring an Athlete of the Month throughout the school year. The requirements for the honor include the f
ollowing – A 12th-grade student, good academic standing, Crookston and surrounding community applicants, possess the following Choice Therapy Values listed below.
– Teamwork (Flexible in the ability to help and contribute to a team; willing to help others, utilize and work with all team members to better others and self; develop and build relationships; engage in community involvement.
– Compassion – (Concern/caring for others; showing kindness and willingness to help others; having a positive interaction with all individuals; desire to serve others; developing a positive and meaningful relationship with others.
– Integrity – (Accountability towards actions; trustworthy; dependable; responsible decision making; honesty; following moral principals.
Nominations and applications are submitted by teachers, coaches, fans, family, or friends. Applications must include the Athletes name, phone number, address, birth date, email, parent(s) name, high school, sports played, and how they possess the Choice Therapy Values. Upon notification as a winner, a parent/guardian must sign a waiver acknowledging the release of the winner’s name and photo to social media as well as media outlets for recognition.
