The East Grand Forks Police Department today released complaint documents against Cadence Marie Zastoupil-Carnes. Zastoupil-Carnes is facing ten felony counts for aggravated forgery and theft by false representation. The complaint alleges that Zastoupil-Carnes forged medical opinion forms and a letter on behalf of herself and her husband, Brent Allen Carnes. The forged documents were used to collect $17,840 in welfare benefits from the Supplemental Nutrition Assistance Program (SNAP) and cash benefits from 2014-2018.
The complaint states that in an interview with police Zastoupil-Carnes admitted to copying and forging a few forms, although she did not remember much about a number of the documents she was shown. In executing a search warrant on Zastoupil-Carnes home last fall, police discovered several documents related to the alleged forgery. Those include a doctor’s stamp on several incomplete forms and blank Altru letterhead. There were also several letters, some containing signatures, that purported to be written by a doctor at Altru. The doctor in whose name and stamp Zastoupil-Carnes is alleged to have forged denied signing, or writing the documents he was shown.
The complaint states that Zastoupil-Carnes was summoned to appear in Polk County District Court Friday morning. The accused pled not guilty, and her next court appearance is scheduled for August 6 at 9:00 a.m.
