POLK AND NORMAN-MAHNOMEN PUBLIC HEALTH COMMUNITY HEALTH ASSESSMENT SURVEY CLOSES IN ONE WEEK

Polk County Public Health (PCPH) and Norman-Mahnomen Public Health (NMPH) are still asking for adults living in Polk, Norman, and Mahnomen County to complete the 2022 Community Health Assessment (CHA) survey.

“Your local voice matters, and we need your input to be successful. This online survey will ask basic questions about your health and quality of life, access to healthcare, what makes being healthy easy or difficult, and how well our communities are supporting your health and mental well-being,” said Sarah Reese Polk-Norman-Mahnomen Community Health Services Administrator/PCPH Director. “The information collected will help us identify strengths and opportunities to improve overall health and wellness in Polk, Norman, and Mahnomen County,” said Sarah Kjono, NMPH Director.  

The online survey is available until July 30, 2022. Your survey responses will remain anonymous and take approximately 10-15 minutes to complete. To be entered into a drawing for one of five $25 gift cards, simply insert your phone number at the end of the survey.

The preferred method of completion is taking the survey online at the following link: https://bit.ly/3OcbNYk, which is also posted on our public health websites. Those without computer or internet access can stop at a Polk County Public Health or Norman-Mahnomen Public Health office to get a copy or call/text 218-521-8348 to have a survey mailed directly to you.
