Polk County Included in Federal Disaster Declaration

As a result of the severe summer storms, tornadoes, and straight-line winds during the Incident period of June 15 to July 11, 2018, Polk County is one of the 27 Counties and 3 Tribal Nations to be included in the Federal Disaster Declaration, number 4390.
This declaration is for the FEMA Public Assistance Program, along with the FEMA Hazard Mitigation Program.
The next step in the process is an Applicant Briefing Meeting, which will be conducted by the state of Minnesota to explain the next steps in the recovery process.  All eligible applicants for this declaration must attend this meeting and fill out a Request for Public Assistance (RPA) form.
The completed forms are then given to the FEMA Public Assistance staff that will be working on our declaration.  FEMA will use the information on this form to contact the eligible applicants, to start gathering the information to write Project Worksheets (PW’s), and start the funding process.
Polk County Emergency Management will be holding the Applicant Briefing Meeting for all the affected agencies at Polk County Sheriff’s Office, 600 Bruce Street, Crookston MN in the Emergency Operations Center.  This meeting is scheduled for Tuesday, October 2, 2018, at 2:00 pm.
Those attending will need to have paperwork/documentation of your losses; this includes bills and pictures of your damages with you when you arrive.  Also, be sure that you have the names, phone numbers, and emails of the primary and alternate contact persons for your agency.  Personnel from the State of Minnesota and FEMA will be on hand to explain the procedure and answer questions for this event.
