Nurses, nursing assistants, therapists, and therapy assistants bring services to the homes of the clients they serve in Polk, Pennington, Red Lake, Marshall, and Norman counties.

Coordinated, Professional Care Home Care clients can access on-call nurses 24 hours a day, including weekends and holidays. Services include Skilled Nursing Care: Nurses assess the client’s needs in the home, coordinate all patient care, obtain samples for laboratory testing, set up and monitor medications, and provide health education and treatments requiring their skills, such as dressing changes, IV antibiotics, and nutrition.
Aide Services: Aides, under the supervision of an RN, provide personal care such as bathing, assistance with exercise, safe transfers and ambulation, and other duties assigned by the nurse.
Physical Therapy (PT): PT staff help clients enhance their healing by promoting independence in mobility. They work to restore or strengthen muscle function through exercise and education using special equipment and treatments to improve quality of life.
Speech Therapy: Speech and language pathologists assess communication skills, evaluate communication problems, and, through appropriate treatment, assist clients in improving communication or swallowing abilities.
Occupational Therapy (OT): OT staff work to increase independence with daily activities such as dressing, bathing, and more. Therapists also provide recommendations for adaptive equipment and home modifications to promote safety in the home.
The cost of services varies depending on individual needs. Medicare, Medicaid, private insurance, Worker’s Compensation, veteran’s benefits, county programs, and private payment may cover Home Care services. 365 Days a Year Customized Living Services is offered at Oak Court Apartments in Crookston. It provides services that help clients stay healthy and safe while living independently.
This program, which operates 365 days a year, features services tailored to the individual needs of each resident, including-
– On-site staff 12-16 hours per day
– Registered nursing on-call
-Assistance with meal preparation
-Medication; set-ups, reminders, and administration
-Treatments and exercises
-Housekeeping
-Laundry
-Safety checks, Personal care
-Socialization
A registered nurse supervises all Customized Living services provided to clients. The nurse is on-site frequently to monitor medication response and request changes when needed.
The Customized Living program includes services for a fixed monthly fee depending on the individual’s needs.
